If you are not sure whether you have an HMO or a PPO plan, please contact your insurance company before your scheduled appointment. If your coverage is an HMO, you will need to obtain an authorization from your referring doctor or your Primary Care Physician and bring it with you to your appointment. If you do not see your insurance company listed above, please call us at (650) 650-324-9200 for assistance. Even though we may not be contracted with your specific insurance plan, we will make every effort to reach an agreement with your carrier on a case-by-case basis. In some situations, we are able to secure payment to allow you to see a Palo Alto Eye Group provider. Our list is subject to change even though we attempt to keep it as up to date as possible.
